Penile curvature (induratio penis plastica): causes and treatment
Penile curvature—medically known as induratio penis plastica (IPP) or Peyronie’s disease—refers to a bend in the penis during an erection. This condition is often accompanied by pain and can be distressing for both the affected man and his partner.
Prevalence
Penile curvature is by no means rare: an estimated 10% of the male population is affected. Although the curvature can occur in any direction, an upward bend is the most common.
Two forms are distinguished:
- Congenital curvature: Caused by developmental abnormalities during penile growth and present from birth
- Acquired curvature (IPP): Usually occurs after the age of 40 and is the significantly more common form
Causes of acquired penile curvature
The exact causes have not yet been fully researched, but several contributing factors are known:
Micro-injuries and scarring
Small, often unnoticed injuries to the penile tissue during sexual intercourse can trigger inflammatory reactions. These lead to scarring and calcium deposits in the erectile tissues. The scarred tissue loses its natural elasticity—during an erection, it stretches less than the surrounding healthy tissue, causing the curvature.
Genetic factors
A family history of the condition can increase the risk. Men with certain connective tissue disorders (e.g., Dupuytren’s disease) are also more frequently affected.
Medications
In rare cases, certain medications are discussed as possible triggers.
What are the symptoms of penile curvature?
Typical signs include:
- Visible curvature of the penis during an erection
- Pain during an erection, especially in the early stages
- Palpable hardening (plaques) on the shaft of the penis
- Difficulty during sexual intercourse due to the altered shape
- Shortening of the penis in some cases
